Gene Therapy

The basic physical and functional unit of heredity is the genes. It is a specific sequence of sources that programs instructions on how to produce proteins. Although genes gets a lot interest because of lack of open understanding of its nature, it is the protein that is the main element that perform most of life functions and even consists of the majority of the cellular structures. When the genes are changed, the programmed or encoded proteins become unable to accomplish the usual function expected from it and genetic disorders arise.

Gene therapy is the replacement of genetic material to an impaired cell. It then corrects the flaw in the genome of a patient. Gene therapy can also be applied to a developed lifetime disease of an individual such as cancer or recurring infection. It gives a specific trait or attribute to the cell giving it strength to combat the disease.

Generally, gene therapy is normally used in acute diseases when the cells of a particular organ in the body cannot function the normal way because it does not have sufficient required protein to perform a specific bodily task. In order to replace the faulty protein, a gene transfer vector or a gene transfer agent is altered so that it contains the gene that encodes for this protein. The altered vector is then administered to the patient. The gene transfer vector delivers the altered gene to the cell, which in turn, the cell’s mechanism converts the healing gene to correct the problem. In essence, it fixes the malfunctioning cell.

To summarize, gene therapy is a procedure for correcting defective genes that are responsible for the development of a disease. There are several approaches that maybe used in correcting faulty genes:

* The most common is the insertion of a normal gene into a non-specific location inside the genome to replace the non-functional gene.

* An abnormal gene is being substituted by a normal gene using homologous recombination or DNA crossover.

* The abnormal gene is repaired by undoing the genetic damage that happened long time ago. It is also called selective reverse mutation, which returns the gene to its normal function.

The gene transfer vector or a gene transfer agent, which is the carrier to deliver the therapeutic gene to the patient’s target cells is basically a virus that has been altered genetically to carry a normal human DNA. Many scientists tried to take advantage of this discovery and manipulate the virus genome to aid in delivering the healing genes and remove the disease causing ones.

Some of the few different types of viruses that are used as gene therapy vectors are retroviruses, adenoviruses, adeno-associated viruses and herpes simplex viruses.

There are also factors that have kept gene therapy from becoming the perfect treatment for genetic diseases. Some of the discovered factors are:

* Risk of viral vectors that once inside the patient’s body, it may recover its ability to cause disease.

* Multigene disorders

Thoracic outlet syndrome

Thoracic outlet syndrome cannot be described as one condition but is the agreed name for a variety of symptoms which are explained by being assumed to be due to compression of the blood vessels and nerves as they go through the area called the thoracic outlet. The outlet is made up of a triangle the boundaries of which are the scalene muscles, the first rib and the collar bone or clavicle, through which the nerves and vessels go to get to the axilla and then to the arm. Patient diagnosis is very difficult in this field and little agreement exists about the condition.

The wide variation in signs and symptoms of sufferers with thoracic outlet syndrome and the absence of any test to confirm or deny its presence means that correct diagnoses of patients with thoracic outlet syndrome is difficult. The numbers of people who suffer from this syndrome is as a consequence not clear although it is known that higher numbers of women occur in this group, especially if they have poor muscle tone and posture.

The nerves and blood vessels travel in what is termed a bundle, moving down from the cervical spine and towards the arm, going through three, mostly triangular spaces, on the way. Compression of the bundle can occur in any of the three triangles, which are already small when the arm is by the side, reducing further in size as the arm moves into certain postures. Postures which increase the tightness of the spaces are used as diagnostic tests to figure out which structures are causing the compression and which are being compressed. Doctors and physiotherapists test by placing the limb of the patient in a specific posture known to be stressful and asking them to perform a repeated muscle action such as fist clenching. This heightens the demand on the blood supply or neurological control required.

The repetitive movement of the shoulder towards the ends of its ranges makes the onset of thoracic outlet syndrome more likely, increasingly so if shoulder abduction (moving the arm out to the side) and outward rotation are involved at end ranges. A common occurrence is for swimmers to complain of pain during their stroke and this should raise the suspicion of thoracic outlet problems. Repetitive shoulder movements towards the end of the available movement make this more likely to occur in many sports or activities. Symptoms may present as neurological difficulties or as problems connected with blood supply to the arm.

Thoracic outlet syndrome presents differently due to whether the compressed structures are the blood vessels, the nerves or both together. The level of pain and disability involved can vary from mild to severe, with symptoms continuous or intermittent. The normal presentation groups are one whose symptoms are not clear or specific, the vascular group and the neurological group. Compression of the main vein or artery in the arm does not occur commonly and perhaps most often in young athletes who perform excessive overhead throwing.

If the arterial flow is disrupted the arm can change colour, there can be pain on muscle use due to their not getting enough blood and an overall pain in the hand and the arm. Mild onset is typical as blood can often get round a blockage, but when the block is large patients attend for medical review independently. Thoracic outlet syndrome from neurological compression involves compression of some of the brachial plexus, a nerve crossroads in the neck which supplies the arms. Nerve compression does not usually occur alone but presents with awkwardness holding a ball or a racket and loss of muscle bulk in the small hand muscles.

Neurological compromise may also cause pins and needles or loss of feeling, with some reports of pain but this tends not to be a major issue. Overhead actions with the arm repetitively tend again to be the aggravating factors. The third group is the contentious one, with a large number of patients who complain of pain in the neck, shoulder blade and arm. Often starting after an accident of some type, this kind of pain is not well understood and there is little medical agreement as to whether this is thoracic outlet syndrome or not.

Headaches have been around from the beginning of time. Well, as far as we know. However, it’s also been believed that headaches were just a fact of life. You get one and you deal with it, that’s it. You can possibly take some over the counter medication, but if that doesn’t work, there’s nothing you can do. The fact is, that the treatment for headaches really has expanded greatly. Whether you get migraines or tension headaches (not all headaches can be reduced by lifestyle changes, it’s important to point out), you can try to do things to help with the pain and how often it knocks you out of the game. Temporomandibular Joint Disorder

Many people suffer from headaches and other head related pains, such as neck ache, jaw pain and even backache. Postpartum Depression Signs

Postpartum depression can exhibit different signs in different women. Almost 80% of mothers can experience what are called “baby blues” or postpartum blues, but this condition is mild and lasts only a few days or weeks after you have given birth.

If you are experiencing “baby blues“, you may cry for long periods of time, without any apparent reason. You may also feel particularly anxious or sad.

However, postpartum depression is different. PPD is much more severe than baby blues, and needs treatment; if you do not get treatment and you are suffering from postpartum depression, it could be dangerous both to you and your baby.

If you are suffering from postpartum depression, you could actually be feeling “high”, or you could experience extreme low moods. You can suffer from extreme feelings of helplessness or fatigue. Oftentimes, postpartum depression is treated with psychotherapy in antidepressants, and these treatments are very effective.

However, if you do not treat your PPD, you can go on to suffer from postpartum psychosis; this is a severe form of postpartum depression and will need immediate medical attention. Postpartum psychosis is extremely rare, but if you have it, you can hallucinate, feel extremely paranoid, and be delusional. You could risk harming yourself or your baby, and may even go on to commit suicide.

If you are a new mother and you experience any of the following symptoms, you should seek immediate help, especially if symptoms are persistent.

These include:

  • Feeling extremely anxious
  • Having extreme mood swings
  • Having extended bouts of crying
  • Having no ability to concentrate
  • Being overly irritable or sad
  •  Feeling shame, guilt or inadequacy persistently
  • Having memory problems
  • Having difficulty bonding with your baby
  • Withdrawing from friends and family
  • Feeling overly angry, for no apparent reason
  • Having trouble sleeping
  • Having urges to harm yourself or your baby, and even thoughts of suicide

If you are feeling any of these symptoms yourself, or if you see them in a loved one, it is very important to seek mental attention immediately. There are indeed hormonal factors that can contribute to depression, and these are unique to women, especially pregnant women or new mothers, so it is very important to identify any stress triggers as well.

Among the causes of PPD may be that in addition to the hormonal swings, and after giving birth, extreme fatigue and the overwhelming responsibility a new baby brings can cause significant anxiety and stress.

Many new moms may feel the need to be the “perfect mom”.

Some signs are indeed easy to see, but others may not be; for example, there may be a history of depression, birth-related difficulties, hormone swings caused by giving birth, financial problems, inadequate support from family or friends, immaturity (especially if the mother or parents of the new child are particularly young), or even drug and substance abuse.

These signs cannot be ignored and should be addressed immediately.

Some mothers may indeed show signs of having “baby blues” in the beginning, but they may persist and become more severe overtime.

If the new mother experiences urges whereby she wants to harm herself and/or her child, or if she loses interest in things that used to engage her, these are things that should be noticed either by the new mother in question or by friends and family.

Treatment for postpartum depression is necessary, and it should be started as quickly as possible. If you are just experiencing normal “baby blues”, for example, you should get over this in a couple of weeks. If you continue to experience depression, however, consider the fact that you or the new mother you are observing may have postpartum depression, and seek treatment immediately.

Stop Sinus Pain
